NP, just measure the dimension and let us have it when you got the new one. I have a good scan I just need the dimension and maybe the patient number format (just the format, no need for your number) (e.g 4134132 is 7 numbers, AA1133A is 2 letters 4 numbers 1 letter)

Just take a picture or scan it and then fold it and put the thing into your wallet with cash so it'd be always with you. Even if it got lost you'd have a digital copy on your phone
